TACOMA, Wash. —

Western State Hospital is investigating after a 44-year-old patient hanged himself.

The Pierce County medical examiner's office says Johannes Helgeson died Tuesday at St. Clare Hospital. He was found hanging Sunday at the psychiatric hospital.

The News Tribune reports (http://is.gd/FftZ7L ) that hospital CEO Ron Adler confirms, in his words, "we did have a death, and it was a completed suicide." He says the hospital is forming a team to investigate.

Adler declined to provide any additional details on the circumstances.
