A federal report says a worker was wearing an inadequate safety harness before he was fatally injured in an Idaho silver mine.

Brandon Gray was trying to dislodge a jammed rock bin at the Lucky Friday Mine on Nov. 17 when he was buried in the rock. The Spokesman-Review reports (http://bit.ly/MKPUKs) the 26-year-old Gray died two days later.

A new federal Mine Safety and Health Administration report says the harness Gray was wearing was designed to arrest an unobstructed fall and that Gray wasn't moving fast enough to cause the lines to lock. The report also says that neither Gray nor his co-worker who survived received safety training before being assigned to work in the bin.

The mine has been temporarily shut down after a series of accidents last year.
